Job Description

Take your Nurse Practitioner expertise into the community with a dynamic per diem role focusing on home health risk assessments. This position provides meaningful patient interactions by delivering in-home evaluations for Medicare Advantage members across Henderson County, TN, including visits in Lexington, Parkers Crossroads, and Sardis. Youll have the opportunity to create your own schedule with flexible shift availability throughout the week.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Control your own work schedule
  • Choose minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Gather and document detailed patient medical histories, current conditions, and medication lists
  • Provide valuable education to members regarding their diagnoses and treatments
  • Recognize and address any gaps in care, making recommendations for follow-up
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board certified through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited program
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience preferred

Your commitment and expertise will be recognized with $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
