Advance your nursing career by providing essential care directly to patients in their homes across Henderson County, TN. In this dynamic per diem opportunity, you will conduct home health risk assessments for Medicare Advantage plan members, making a direct impact on the wellbeing of individuals residing in Lexington, Parker's Crossroads, Sardis, and surrounding areas.
Schedule:
- Flexible shifts available throughout the week: Monday through Sunday
- Design your own work schedule to fit your lifestyle
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual home health risk assessments, typically lasting about 45 minutes per visit
- Review and evaluate patients prior and current medical history, medical conditions, and medication management
- Educate patients on their health conditions and medications
- Identify potential gaps in care and make informed recommendations for improvement
- Visits do not require creating treatment plans or prescribing medications
- Visits may include standard annual health assessments or post-discharge check-ins
Qualifications:
- Current Nurse Practitioner License in TN
- Board Certification through AANP or ANCC
- Graduate of an accredited nursing program with an MSN degree
- Active CPR certification (BLS)
- At least 6 months of recent NP experience is preferred
