Deliver exceptional patient care in a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments in McMinn County, TN. This flexible role allows you to create your own schedule with shifts available between Monday and Sunday, offering a unique chance to balance professional commitments and personal responsibilities. Youll be traveling to the homes of Medicare Advantage plan members in Athens, Etowah, Englewood, and surrounding areas, providing vital annual health assessments and post-discharge visits.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Commit to a minimum of weekly or bi-weekly hours
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review prior and current medical history, existing conditions, and medication regimens
- Educate patients on their health conditions and prescriptions
- Identify gaps in care and advise actionable solutions to address them
- Visits are focused on assessments; no treatment planning or prescriptions required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
