Job Description

Take on a rewarding per diem opportunity using your advanced practice skills to improve patient outcomes through home health risk assessments. In this flexible role, youll visit Medicare Advantage plan members in their homes across Adams County, including areas such as Decatur, Berne, and Geneva. Each home health assessment typically takes about 45 minutes, focusing on annual health checkups or post-discharge visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your lifestyle
  • Commitment required on a weekly or bi-weekly basis

Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, medical conditions, and medication records
  • Educate members on their health conditions, medications, and preventive care
  • Identify care gaps and recommend solutions
  • No prescriptive authority or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board certification via AANP or ANCC
  • Possess a Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR (BLS) certification
  • A minimum of six months experience as a Nurse Practitioner is preferred
