Advance your career as a Nurse Practitioner specializing in home health by providing essential risk assessments for Medicare Advantage members. This per diem opportunity is ideal for skilled professionals seeking autonomy, variety, and flexible scheduling within Adams County, IN. You will work with patients in diverse communities, including Decatur, Berne, and Geneva, and play a vital role in helping individuals understand and manage their health.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose from minimum weekly or bi-weekly hourly commitments
Responsibilities:
- Perform comprehensive annual risk assessments in patients homes, spending approximately 45 minutes per evaluation
- Review and evaluate past and current medical history, chronic conditions, and medication usage
- Deliver clear education to patients regarding their diagnoses, treatment options, and medication regimens
- Identify gaps in healthcare and recommend steps for bridging those gaps
- Focus exclusively on assessment and educationno diagnosis, treatment planning, or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board certified through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current Basic Life Support (BLS) CPR certification
- At least 6 months of recent experience as a Nurse Practitioner is strongly preferred
Candidates are compensated between $80 and $90 per evaluation, including a mileage stipend and pay for administrative time.
