Advance your clinical practice in home health with a per diem role focused on risk assessments for Medicare Advantage members across Miami County. This opportunity is ideal for Nurse Practitioners seeking flexible scheduling and autonomy, serving patients in Troy, Piqua, Tipp City, Union, and other local communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to design your own work schedule
- Minimum weekly or bi-weekly hourly commitment based on your preference
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes.
- Review and document detailed medical, medication, and prior health histories.
- Provide patient education on chronic conditions, medications, and preventive care strategies.
- Identify gaps in care and recommend solutions to optimize health outcomes.
- Visits may include annual health assessments or post-discharge follow-ups (approximately 45 minutes each).
- No requirement for developing treatment plans or prescribing medications.
Qualifications:
- Active Nurse Practitioner (NP) license in Ohio.
- Board certification through either AANP or ANCC.
- CPR certification (BLS).
- Master of Science in Nursing (MSN) from an accredited institution.
- At least 6 months of current NP experience preferred, with a background in home health an advantage.
