This opportunity is ideal for Nurse Practitioners seeking flexible scheduling and autonomy, serving patients in Troy, Piqua, Tipp City, Union, and other local communities.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to design your own work schedule
• Minimum weekly or bi-weekly hourly commitment based on your preference

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes.
• Review and document detailed medical, medication, and prior health histories.
• Provide patient education on chronic conditions, medications, and preventive care strategies.
• Identify gaps in care and recommend solutions to optimize health outcomes.
• Visits may include annual health assessments or post-discharge follow-ups (approximately 45 minutes each).
• No requirement for developing treatment plans or prescribing medications.

Qualifications:

• Active Nurse Practitioner (NP) license in Ohio.
• Board certification through either AANP or ANCC.
• CPR certification (BLS).
• Master of Science in Nursing (MSN) from an accredited institution.
• At least 6 months of current NP experience preferred, with a background in home health an advantage.
