This position is designed for skilled Nurse Practitioners who excel in providing comprehensive, patient-centered care in a home setting. Each evaluationlasting approximately 45 minutesoffers a unique chance to connect with Medicare Advantage plan members, supporting their overall wellness and addressing potential gaps in care. Shifts are flexible between Monday and Sunday, and you have the autonomy to create your own schedule, with minimum weekly or bi-weekly hourly commitments.

Core Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan participants
  • Evaluate prior and current medical history, conditions, and medication regimens
  • Educate members on their health status, medications, and disease management
  • Identify gaps in care, recommending actionable steps for resolution
  • No requirement for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Current CPR certification (BLS)
  • Board certification from AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of current experience as a Nurse Practitioner preferred

This role offers $80$90 compensation per risk assessment evaluation completed, with an additional mileage stipend and administrative time included.
