Job Description
Experienced Nurse Practitioners are needed to perform comprehensive home health risk assessments for Medicare Advantage plan members across Genesee County, MI. This per diem position offers flexibilitychoose your own shifts between Monday and Sunday and work in Flint, Grand Blanc, Burton, and surrounding areas, with minimal weekly or bi-weekly hourly commitments to match your schedule.
Key Responsibilities:
- Complete home health risk assessments (approximately 45 minutes each) during annual visits or post-discharge check-ins
- Assess prior and current medical history, identify existing conditions, and review medication regimens
- Educate patients regarding their medical conditions and medication usage
- Identify gaps in healthcare and provide recommendations to address them
- No creation of treatment plans or medication prescribing is required
Schedule:
- Create your own scheduleworkdays are available Monday through Sunday
- Flexible shifts for optimal work-life balance
- Minimum commitment on a weekly or bi-weekly basis
Qualifications:
- Active Nurse Practitioner License in Michigan
- Board Certification through AANP or ANCC required
- MSN degree from an accredited nursing program
- Current BLS/CPR certification
- At least 6 months of recent Nurse Practitioner experience preferred
- Reliable transportation to travel within Genesee County
