Take on a meaningful role delivering care and health assessments as a Nurse Practitioner in a per diem home health setting. In this position, you will complete comprehensive risk assessments for Medicare Advantage plan members based throughout Osceola County, including Kissimmee, St. Cloud, and Poinciana. Home visits average 45 minutes and may include annual assessments or post-discharge check-ins, all with an emphasis on patient education and gap identification without the responsibility for treatment planning or prescribing.
Schedule:
- Flexible shift options offered Monday through Sunday
- Self-managed scheduling to accommodate your availability
- Minimum weekly or bi-weekly hourly commitment as agreed
Key Responsibilities:
- Perform detailed annual home health risk assessments for Medicare Advantage members
- Collect and review medical history, current conditions, and medication records
- Educate patients on health conditions and appropriate medication use
- Recognize and document gaps in care, offering clear recommendations for next steps
- Focus assessments on identification and educationno treatment plans or prescribing involved
Qualifications:
- Licensed as a Nurse Practitioner in the state of Florida
- Holds current CPR (BLS) certification
- Board Certified by either the AANP or ANCC
- Completion of an MSN degree from an accredited nursing school
- At least 6 months of recent NP experience preferred
