Deliver essential care in the community as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity lets you set your own pace, as you travel throughout Glynn County, evaluating Medicare Advantage plan members in their homes. The work covers a range of locations, including Brunswick and the Golden Isles, with visits taking approximately 45 minutes each.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Create your own weekly or bi-weekly schedule
- Minimum hourly commitment per week or every two weeks
Key Responsibilities:
- Perform comprehensive annual and post-discharge health risk assessments in patients homes
- Review and assess prior and current medical history, ongoing conditions, and current medication regimens
- Educate patients about their health conditions and medications
- Identify any gaps in care and provide actionable recommendations
- No prescribing or treatment planning required
Qualifications:
- Active and unencumbered Nurse Practitioner license in Georgia
- Board certification by AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
Earn between $80 and $90 per completed evaluation, with additional mileage stipend and compensation for administrative time. This position offers the autonomy to manage your own schedule while making a meaningful impact on patient care.
