Deliver valuable healthcare in your community as an experienced provider performing home-based risk assessments. This flexible per diem opportunity is ideal for Nurse Practitioners seeking meaningful work that fits their lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign shifts around your availability
- Minimum weekly or bi-weekly hour commitment required
Responsibilities:
- Conduct thorough home health risk assessments for Medicare Advantage plan members, with each assessment taking approximately 45 minutes
- Evaluate patients past and current medical histories, conditions, and medication usage
- Offer education related to medical conditions and medication adherence
- Identify gaps in care and provide actionable recommendations to enhance patient outcomes
- Visits may include annual health assessments or post-discharge follow-ups
- Travel within assigned areas in Oglethorpe County, with potential scheduling in Lexington, Crawford, Maxeys, and surrounding communities
- Documentation and administrative requirements are included, but there is no expectation for you to develop treatment plans or prescribe medications
Qualifications:
- Active GA Nurse Practitioner License
- Master of Science in Nursing (MSN) from an accredited institution
- Board certification from the AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- Minimum six months of Nurse Practitioner experience preferred
- Strong communication and teaching skills, with attention to accuracy and detail
