Experienced Nurse Practitioners are invited to provide high-quality home health risk assessments on a flexible, per diem basis within Cocke County, TN. In this rewarding role, you will visit Medicare Advantage plan members in their homes throughout Newport, Parrottsville, and the surrounding communities, conducting comprehensive annual or post-discharge assessments.
Schedule:
- Flexible shifts available Monday through Sunday
- Opportunity to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough home health risk assessments for Medicare Advantage health plan members
- Evaluate patients prior and current medical histories, ongoing conditions, and medication regimens
- Deliver patient education regarding conditions and medications to increase understanding and compliance
- Identify potential gaps in care and provide recommendations to address them
- No management of treatment plans or prescribing requiredyour focus will be comprehensive assessment
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS (CPR) certification
- Board certification from AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience is preferred
This position offers competitive compensation, with $80$90 per evaluation along with a mileage stipend and payment for administrative time.
