Job Description

Make a meaningful impact by delivering patient-centered care through home health risk assessments for Medicare Advantage plan members across Marion County, OR. As a Nurse Practitioner in this per diem role, youll conduct comprehensive evaluations, helping to bridge gaps in care while empowering patients with knowledge about their health.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Options for minimum weekly or bi-weekly hourly commitments

Essential Responsibilities:

• Perform annual health and post-discharge risk assessments at patients homes in areas such as Salem, Keizer, Woodburn, and Hayesville
• Review medical histories, current conditions, and medication regimens during each visit (45 minutes per evaluation)
• Educate patients on their health conditions and safe medication use
• Identify gaps in care and recommend steps for improved health outcomes
• Focus exclusively on assessmentno requirement to create treatment plans or prescribe medications

Qualifications and Experience:

• Active Nurse Practitioner license in Oregon
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least six months of recent experience as a Nurse Practitioner preferred
