Schedule:

• Choose your own shifts between Monday and Sunday
• Design a schedule that fits your lifeminimum hourly commitments on a weekly or bi-weekly basis

Key Responsibilities:

• Perform annual risk assessments in patients homes, typically lasting about 45 minutes per visit
• Review prior and current medical histories, examine medical conditions, and assess medication usage
• Educate individuals on their health conditions and medication regimens
• Identify gaps in care, making thoughtful recommendations for improvement
• Note: No need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Texas
• Board Certification from AANP or ANCC required
• Current CPR/BLS certification
• Masters degree in nursing (MSN) from an accredited institution
• Minimum six months of NP experience preferred, especially in home health
