Job Description
Play a key role in enhancing patient care by performing home health risk assessments as a Nurse Practitioner on a per diem basis. This opportunity allows you the autonomy to create your own schedule, with flexible shifts available Monday through Sunday. Assignments are within Summit County, serving areas such as Cuyahoga Falls, Stow, and Barberton. Each assessment, conducted for Medicare Advantage plan members, averages 45 minutes, and may include annual health evaluations or post-discharge visits.
Schedule:
- Flexible shifts between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members.
- Assess patients prior and current medical history, existing conditions, and medication use.
- Educate patients on their medical conditions and medication regimen.
- Identify care gaps and make evidence-based recommendations to address them.
- No prescribing or treatment plan creation is required.
Qualifications:
- Active Nurse Practitioner license in Ohio.
- Current CPR (BLS) certification.
- Board Certification through AANP or ANCC.
- At least 6 months of current Nurse Practitioner experience preferred.
- MSN degree from an accredited nursing program.
