Job Description

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments in patient homes
  • Review and evaluate patients prior and current medical histories, medication lists, and medical conditions
  • Deliver patient education on medical diagnoses and prescribed medications
  • Identify care gaps and offer actionable recommendations for closing these gaps
  • Focus solely on assessmentsno requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in the state of Ohio
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current Basic Life Support (BLS) / CPR certification
  • At least 6 months of current Nurse Practitioner experience preferred

This position offers competitive earnings$80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
