Advance your clinical skills as a Nurse Practitioner in a dynamic home health environment, providing risk assessments for Medicare Advantage plan members throughout Portage County, OH. Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patient homes
- Review and evaluate patients prior and current medical histories, medication lists, and medical conditions
- Deliver patient education on medical diagnoses and prescribed medications
- Identify care gaps and offer actionable recommendations for closing these gaps
- Focus solely on assessmentsno requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of Ohio
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current Basic Life Support (BLS) / CPR certification
- At least 6 months of current Nurse Practitioner experience preferred
This position offers competitive earnings$80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
