This per diem opportunity offers the flexibility to tailor your work schedule to fit your lifestyle, with shifts available throughout the weekincluding weekends. Choose your hours with a minimum weekly or bi-weekly commitment, and complete rewarding visits in various communities within Montgomery County, TN.
Schedule/Hours
- Flexible shifts available Monday through Sunday
- Self-scheduling with the ability to create your own workweek
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities
- Perform in-home health risk assessments, primarily for Medicare Advantage plan members
- Assess past and present medical history, conditions, and medications
- Deliver patient education regarding medical conditions and prescribed medications
- Identify care gaps and make actionable recommendations to address them
- Visits typically last around 45 minutes per member; no treatment planning or prescribing required
Qualifications
- Valid Nurse Practitioner License in Tennessee
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN degree from an accredited institution
- At least 6 months of current NP experience preferred
Your compensation includes $80-$90 for each completed evaluation, with additional stipends for mileage and administrative time.
