Take on a dynamic per diem opportunity as a Nurse Practitioner providing home health risk assessments throughout Nez Perce County. In this essential role, you'll visit Medicare Advantage plan members in their homes, conducting comprehensive evaluations that help ensure quality care while delivering a more convenient patient experience.
Schedule:
- Flexible shifts available Monday through Sunday
- Customize your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform annual home health risk assessments, which may include post-discharge visits, in the homes of Medicare Advantage members across Lewiston, Lapwai, Peck, and nearby areas
- Review and evaluate patients prior and current medical history, conditions, and medication use
- Educate patients about their health conditions and medication regimen
- Identify any gaps in care, providing clear recommendations for follow-up with primary providers
- Document findings without the need to create formal treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Idaho
- Active Board Certification from AANP or ANCC
- Current BLS CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
In recognition of your expertise and dedication, compensation for this per diem role is provided at $80-$90 per completed evaluation, including an additional mileage stipend and paid administrative time.
