Make a meaningful impact in the community by performing in-home health risk assessments for Medicare Advantage plan members throughout Cibola County. This per diem opportunity is perfect for skilled Nurse Practitioners seeking flexibility and variety in their work, as youll provide essential healthcare services across diverse settings within Grants, Milan, Acoma Pueblo, and beyond.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Create your own schedule
- Meet minimum weekly or bi-weekly hourly requirements
Key Responsibilities:
- Complete thorough annual and post-discharge health risk assessments during each home visit (approximately 45 minutes per evaluation)
- Assess patients prior and current medical history, medical conditions, and medication usage
- Deliver clear education to patients on their conditions and medication regimens
- Detect gaps in care and offer recommendations to improve overall health outcomes
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New Mexico
- Board certification via AANP or ANCC
- Current CPR/BLS certification
- MSN from an accredited institution
- Minimum of six months Nurse Practitioner experience preferred
