Job Description

Deliver essential care in the community with a per diem opportunity for an experienced Nurse Practitioner specializing in home health assessments. As part of this role, you will perform comprehensive in-home risk assessments for Medicare Advantage plan members throughout Allegany County, including Wellsville, Alfred, Cuba, and Caneadea. Each evaluation typically takes about 45 minutes and may include annual health assessments or post-discharge visits, giving you the chance to make a direct impact on patient outcomes and quality of life.

  • Flexible shifts available Monday through Sunday

Essential Qualifications

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing institution
  • Current CPR (BLS) certification
  • At least 6 months of recent NP experience preferred
  • Prior experience in home health or risk assessment is a plus

Primary Responsibilities

  • Conduct annual Medicare Advantage home health risk assessments in patient residences
  • Review and document prior/current medical history, medication use, and existing health conditions
  • Provide clear education to patients on their health conditions and medication regimens
  • Identify care gaps and recommend strategies for improved health outcomes
  • Note: This role does not involve prescribing medication or creating treatment plans
