Job Description
Experienced nurse practitioners are sought to conduct home health risk assessments for Medicare Advantage members in Allegany County, NY, including locations such as Wellsville, Alfred, Cuba, and Caneadea. This per diem opportunity offers highly flexible scheduling, allowing you to design shifts around your life. Visits last approximately 45 minutes each and may involve annual health assessments or post-discharge check-ins, without requirements for treatment planning or prescribing.
Schedule
- Flexible shifts available Monday through Sunday
- Create your own schedule based on your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Perform home-based risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical histories, chronic conditions, and medication histories
- Educate patients regarding their health conditions and prescribed medications
- Identify care gaps and make recommendations to facilitate resolution
- Visits are focused on assessment and educationno development of treatment plans or prescribing is needed
Qualifications
- Current Nurse Practitioner license in New York
- Active BLS CPR certification
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing program
- Minimum of six months NP experience preferred
Compensation for each evaluation ranges from $80-$90, in addition to a mileage stipend and compensation for administrative time.
