Job Description

Make a meaningful impact by providing essential risk assessments as a Nurse Practitioner in a home health setting. This per diem position involves traveling to patients' residences throughout Crystal Lake, Algonquin, Lake in the Hills, and surrounding communities within McHenry County. Your expertise will help ensure comprehensive, individualized healthcare to Medicare Advantage plan members in the comfort of their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan participants
  • Evaluate prior and current medical history, ongoing medical conditions, and medication usage
  • Educate individuals and caregivers on health conditions and medication management
  • Identify gaps in care and propose actionable recommendations
  • Conduct visits focused solely on assessment (no prescribing or treatment planning required)
  • Provide thorough documentation after each visit

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least six months of nurse practitioner experience preferred
