Advance your career in a flexible, patient-focused role reviewing and supporting the health of Medicare Advantage members in their homes. As a Per Diem Nurse Practitioner specializing in home health, you will have the opportunity to design your own work schedule and provide essential risk assessments throughout Augusta County, including Staunton, Craigsville, and Grottoes.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Evaluate past and current medical history, conditions, and medication regimens
- Educate patients on their medical conditions, recommended interventions, and medications
- Identify gaps in care and make recommendations for improvements
- Visits may include annual assessments or post-discharge follow-ups
- No prescribing or creation of long-term treatment plans required
Qualifications:
- Active Nurse Practitioner license in Virginia
- Board Certification via AANP or ANCC
- CPR certification (BLS)
- MSN degree from an accredited nursing school
- At least 6 months of current NP experience preferred
