This rewarding per diem opportunity focuses on providing comprehensive risk assessments for Medicare Advantage plan members throughout Lake County. Your evaluations could range from annual health assessments to post-discharge follow-ups, with each visit averaging about 45 minutes. Leverage your clinical expertise to drive quality care across diverse communities.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Design your own work schedule
• Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform home health risk assessments for Medicare Advantage members in various locations within Lake County
• Review and evaluate each patients medical history, current conditions, and medications
• Deliver clear education regarding diagnoses and proper medication use
• Identify care gaps and recommend steps to improve overall patient health
• No prescribing or treatment planningfocus is on thorough assessment and guidance

Qualifications:

• Active Nurse Practitioner license for Indiana
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing program
• At least six months of experience as a Nurse Practitioner preferred
