Make a meaningful impact in community healthcare by delivering in-home assessments as a Nurse Practitioner specializing in home health. This rewarding per diem opportunity focuses on providing comprehensive risk assessments for Medicare Advantage plan members throughout Lake County. Your evaluations could range from annual health assessments to post-discharge follow-ups, with each visit averaging about 45 minutes. Leverage your clinical expertise to drive quality care across diverse communities.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule
- Choose your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform home health risk assessments for Medicare Advantage members in various locations within Lake County
- Review and evaluate each patients medical history, current conditions, and medications
- Deliver clear education regarding diagnoses and proper medication use
- Identify care gaps and recommend steps to improve overall patient health
- No prescribing or treatment planningfocus is on thorough assessment and guidance
Qualifications:
- Active Nurse Practitioner license for Indiana
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited nursing program
- At least six months of experience as a Nurse Practitioner preferred
