Nurse Practitioners with experience in home health care are invited to take on a vital and flexible per diem position conducting in-home risk assessments for Medicare Advantage members. This opportunity allows you to work in various locations within Cumberland County, including Crossville, Pleasant Hill, and Crab Orchard. As part of this role, you will play an essential part in ensuring comprehensive, patient-centered care for individuals in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage health plan members
- Evaluate prior and current medical history, conditions, and medication usage
- Educate patients about their conditions and prescribed medications
- Identify potential gaps in care and suggest resolutions
- No prescribing or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license (TN)
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months recent experience as an NP preferred
- MSN degree from an accredited nursing school
