Make a meaningful impact in patient care by providing home health risk assessments for Medicare Advantage plan members throughout Lane County. As a Nurse Practitioner specializing in home health, you will conduct thorough evaluations in patients homes, with each assessment typically taking about 45 minutes. Visits may include annual health assessments or post-discharge checkups. Assignments are scheduled in multiple locations within Lane County, including Eugene and surrounding communities such as Springfield, Elmira, and Cottage Grove. This per diem role offers significant flexibility to support work-life balance.

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Review patients medical history, chronic conditions, and current medications
• Educate patients on managing their medical conditions and proper medication use
• Identify care gaps and make effective recommendations for follow-up
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Oregon
• Board certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months of recent NP experience preferred
