Job Description

Deliver comprehensive patient care in a rewarding per diem role focused on home health risk assessments. As a Nurse Practitioner working with Medicare Advantage plan members across Putnam Countyincluding the communities of Palatka, Interlachen, and Crescent Cityyou will provide critical annual and post-discharge assessments that support member health and wellness.

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Design your own schedule to balance with your lifestyle
  • Set your own minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform thorough home health risk assessments for Medicare Advantage members
  • Conduct annual health evaluations as well as post-discharge visits, averaging 45 minutes each
  • Review and document patient medical histories, existing conditions, and medication use
  • Educate patients on chronic disease management and safe medication practices
  • Detect care gaps and offer practical recommendations to optimize patient outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner (NP) license in the state of Florida
  • Board Certification through AANP or ANCC
  • Masters degree (MSN) from an accredited nursing program
  • Current CPR certification (BLS)
  • At least 6 months of nurse practitioner experience preferred
  • Excellent interpersonal skills, ability to work independently, and comfort with traveling to patients homes
