Step into a dynamic role focused on providing high-quality home health risk assessments for Medicare Advantage plan members in Leon County, FL, including areas such as Tallahassee, Woodville, and Midway. As a Nurse Practitioner specializing in home health, you will leverage your expertise to enhance patient quality of life and ensure continuity of care in a per diem capacity.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to accommodate your availability
• Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

• Conduct comprehensive annual risk assessments in patients homes (approximately 45 minutes per visit)
• Review prior and current medical histories, medical conditions, and medication regimes
• Offer patient education on health conditions and medications to promote self-management
• Identify care gaps; deliver actionable recommendations to address these gaps
• Complete risk assessments without the requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Florida
• Current CPR certification (BLS required)
• Board certification through AANP or ANCC
• MSN degree from an accredited school of nursing
• At least 6 months of current NP experience preferred
