Advance your clinical skills by providing home health risk assessments as a Nurse Practitioner in a dynamic per diem role. This position offers the flexibility to build your own schedule and provides opportunities for independent practice across Leon County, including areas such as Tallahassee, Woodville, and Midway.
Schedule:
- Flexible scheduling available Monday through Sunday
- Create your own shifts
- Minimum weekly or bi-weekly commitment
Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Evaluate medical histories, health conditions, and medication use during each assessment
- Offer meaningful education to patients regarding their medical conditions and prescribed medications
- Identify any existing care gaps and suggest appropriate interventions
- Focus solely on assessments and education; there is no requirement to create treatment plans or prescribe medications
Qualifications:
- Active and unrestricted Nurse Practitioner license in Florida
- Current CPR certification (BLS required)
- Board Certification as an NP via AANP or ANCC
- At least 6 months of experience working as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
This independent, patient-centered position provides compensation of $80 to $90 per evaluation, including a mileage stipend and compensation for administrative time.
