Put your clinical expertise to work by conducting home health risk assessments as a Nurse Practitioner for Medicare Advantage members across Providence County, RI. This per diem position offers a unique opportunity to deliver high-quality care and education right in patients homes in areas such as Cranston, Woonsocket, and Scituate. Schedule:

• Flexible shifts between Monday and Sunday
• Design your schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual health risk assessments in members homes, with each visit averaging 45 minutes
• Review and evaluate medical histories, current health conditions, and medication usage
• Provide clear patient education on chronic conditions and medication management
• Assess gaps in care and provide actionable nursing recommendations
• Visits may also include post-discharge assessments as needed
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in Rhode Island
• Board certification by AANP or ANCC
• Completion of an accredited MSN program
• Active CPR certification (BLS)
• Six months or more of recent NP experience preferred
