Leverage your expertise as a Nurse Practitioner to make a difference in the community through per diem home health visits. You'll conduct comprehensive risk assessments for Medicare Advantage plan members in Kent County, RI, including areas such as Warwick, West Warwick, and Coventry. Assessments typically last around 45 minutes and may involve annual health evaluations or post-discharge check-ins.

Schedule:

• Flexible shifts available Monday through Sunday
• Customize your schedule to fit your availability
• Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage health plan members
• Evaluate both current and past medical history, existing conditions, and medication usage
• Offer patient education about medical conditions and proper medication management
• Identify gaps in care and recommend solutions to address them
• Focus entirely on assessmentsno need to create treatment plans or prescribe medications

Preferred Qualifications:

• Active Nurse Practitioner license in the state of Rhode Island
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing institution
• At least 6 months of recent Nurse Practitioner experience, especially in home health settings, is preferred

For each completed evaluation, youll receive $80$90, plus a mileage stipend and compensation for administrative duties.
