Serve as a pivotal healthcare provider by delivering home health risk assessments for Medicare Advantage members within Cumberland County, TN, including the communities of Crossville, Pleasant Hill, and Crab Orchard. This per diem role offers the ability to tailor your schedule, ensuring work-life balance and autonomy. Typical home visits last approximately 45 minutes, encompassing a range of assessments, from annual checkups to post-discharge evaluations. Expect to be scheduled at various locations within the county during individual shifts, making adaptability and organizational skills key.
Schedule:
- Flexible shifts available seven days a week
- Create your own schedule
- Minimal weekly or bi-weekly hourly commitments as needed
Core Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical histories, conditions, and medication profiles during in-home visits
- Educate patients about their medical conditions, recommended preventive measures, and medication management
- Detect gaps in care and provide thoughtful recommendations to enhance patient outcomes
- No responsibility for developing treatment plans or prescribing medications
Required Qualifications:
- Active and unrestricted Nurse Practitioner license in Tennessee
- Current certification in Basic Life Support (CPR/BLS)
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- Graduation from an accredited nursing program with an MSN degree
- At least six months of recent Nurse Practitioner experience preferred
