Job Description

Seeking a dedicated Nurse Practitioner to conduct home health risk assessments on a per diem basis within Cumberland County, TN. This position offers the flexibility to craft your own schedule, with shifts available any day between Monday and Sunday. Locations for patient visits include Crab Orchard, Crossville, and Pleasant Hill, providing essential healthcare services to Medicare Advantage plan members in their homes.

Schedule:

Key Responsibilities:

  • Complete annual home health risk assessments as part of Medicare Advantage healthcare initiatives
  • Review and evaluate patients' prior and current medical history, health conditions, and medication history during 45-minute in-home visits
  • Educate individuals on disease processes, preventive health, and medication compliance
  • Identify gaps in care and provide actionable recommendations, helping to enhance the overall quality of care
  • No responsibility for creating ongoing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board certification via AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred

Compensation includes $80$90 per evaluation, along with a mileage stipend and pay for administrative time, supporting both the time you spend with patients and the travel required to serve the region.
