Schedule:

• Flexible shifts available any day, Monday through Sunday
• Create your own schedule tailored to your availability
• Meet a minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage members in their homes
• Assess patients prior and current medical history, underlying conditions, and medication lists
• Deliver education regarding disease management and prescribed medications
• Identify gaps in care and support patients with actionable recommendations
• Complete documentation for each assessment, with no treatment planning or prescribing required

Qualifications:

• Valid Nurse Practitioner license in Oklahoma
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• A minimum of 6 months experience as a Nurse Practitioner is preferred
• Masters degree in Nursing (MSN) from an accredited institution

This position offers the flexibility to build your schedule, concentrating your visits where you choose within Wagoner County. You will be compensated $80-$90 per evaluation, with additional stipends for mileage and administrative time.
