Take on a meaningful role providing home health risk assessments for Medicare Advantage members across Muskogee County. In this per diem opportunity, you'll work directly with patients in their residences to support comprehensive care and health education in the community.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule
- Minimum commitment required weekly or bi-weekly (hours based on your availability)
Primary Responsibilities:
- Conduct in-home risk assessments for Medicare Advantage members, averaging 45 minutes each
- Travel to scheduled visits within Muskogee County, including areas such as Muskogee, Fort Gibson, and Warner
- Review past and current medical history, medical conditions, and medication usage
- Educate patients on conditions and prescribed medications
- Identify care gaps and provide actionable recommendations to address them
- Note: No treatment planning or medication prescribing required for this position
Qualifications:
- Valid Nurse Practitioner license in Oklahoma
- Board certified by AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing school
- Minimum 6 months of recent Nurse Practitioner experience preferred
This position offers compensation of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
