Elevate your nurse practitioner career with a flexible per diem opportunity focused on home health risk assessments throughout Muskogee County. Leveraging your clinical skills, you will visit Medicare Advantage members in their homes and provide valuable health evaluations, education, and recommendations to enhance their well-being.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitments, allowing for excellent work-life balance
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Travel to member residences within areas like Muskogee, Fort Gibson, and Warner
- Review and assess patients medical histories, existing conditions, and medications
- Deliver patient-centered education regarding health conditions and treatments
- Identify care gaps and suggest actionable recommendations
- No requirement for treatment planning or prescribing medications
Qualifications:
- Valid Nurse Practitioner License in Oklahoma
- Active CPR certification (BLS)
- Board Certification via AANP or ANCC
- At least 6 months current experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
