A rewarding opportunity is available for an experienced Nurse Practitioner to provide home health risk assessments on a per diem basis within Alleghany County, including Covington, Clifton Forge, and Iron Gate. This vital position focuses on thorough patient evaluations for Medicare Advantage plan members, helping to promote ongoing quality care in the comfort of their homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule with minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual or post-discharge home health risk assessments (approximately 45 minutes per assessment)
• Review and evaluate prior and current medical history, ongoing medical conditions, and medication regimens
• Educate patients on health conditions and prescribed medications to enhance understanding and compliance
• Identify gaps in care and provide actionable recommendations to address these issues
• Strictly no requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Virginia
• Current CPR (BLS) certification
• Board certification through AANP or ANCC required
• Minimum of 6 months' Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited program
