Take on a highly impactful per diem Nurse Practitioner role focused on home health risk assessments in Idaho County. This flexible opportunity invites you to provide care and evaluation for Medicare Advantage plan members across a range of communities, including Grangeville, Cottonwood, and Kamiah. Enjoy a work environment that allows you to design your own schedule, committing to a minimum number of hours weekly or bi-weekly, with shifts available any day from Monday through Sunday.
Schedule:
- Flexible shifts offered MondaySunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments in patients homes, generally completed in about 45 minutes each
- Review and evaluate medical history, current conditions, and medication use
- Educate patients regarding diagnoses and medication management
- Identify care gaps and recommend solutions to improve health outcomes
- No responsibilities for treatment planning or medication prescribing
Qualifications:
- Current Nurse Practitioner license in Idaho
- Completion of an MSN from an accredited nursing program
- Active board certification (AANP or ANCC)
- Current BLS (CPR) certification
- At least 6 months of recent Nurse Practitioner experience preferred
