Experienced Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage members across Jackson County, FL. This per diem role offers flexible scheduling and the autonomy to create shifts that fit your lifestyle. The primary task involves conducting thorough yet efficient risk assessmentstypically lasting around 45 minutes per visitin patient homes within locations such as Marianna, Graceville, and Sneads.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Commitment can be weekly or bi-weekly, based on your availability
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and document prior and current medical history, conditions, and medication use
- Educate members on their health conditions and prescribed medications
- Detect gaps in care and recommend steps to address these needs
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited school of nursing
- Minimum of 6 months recent NP experience preferred
This opportunity features compensation of $80-$90 for each evaluation performed, with additional mileage reimbursement and compensation for administrative responsibilities.
