Job Description

Deliver comprehensive, patient-focused care as a Home Health Nurse Practitioner providing risk assessments throughout Jackson County, FL. In this per diem opportunity, youll visit Medicare Advantage plan members in their homesincluding areas such as Marianna, Graceville, and Sneadsto complete detailed health assessments and offer valuable patient education. The role offers flexible scheduling, allowing you to choose your shifts between Monday and Sunday and tailor your work commitment to your availability.

Schedule/Hours

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum hourly commitment per week or every two weeks

Key Responsibilities

  • Conduct annual home health risk assessments for Medicare Advantage health plan members
  • Review and evaluate patients medical history, ongoing conditions, and medication regimens
  • Provide education to patients on disease management and prescription medications
  • Identify care gaps and recommend appropriate actions to address them
  • Focus exclusively on assessmentsno diagnosis, prescription, or development of treatment plans

Qualifications

  • Active Nurse Practitioner license in Florida
  • Board Certification from AANP or ANCC
  • MSN from an accredited school of nursing
  • Current CPR/BLS certification
  • At least 6 months of recent Nurse Practitioner experience preferred
