A unique opportunity is available for Nurse Practitioners with experience in home health to perform risk assessments for Medicare Advantage members throughout Lane County. This per diem position offers maximum flexibility, allowing you to set your own schedule and select shifts that work best for your lifestyle.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own schedule according to your preferences
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual home health risk assessments in members' homes across Lane County, including Springfield, Elmira, Cottage Grove, and surrounding areas
• Conduct comprehensive evaluations of patients prior and current medical history, medical conditions, and medication profiles
• Deliver education to patients about their health conditions and medication usage
• Identify gaps in care and make recommendations to improve overall patient health outcomes
• Focus exclusively on evaluation and educationno need to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in the state of Oregon
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months of current NP experience is preferred
• Master of Science in Nursing (MSN) from an accredited institution

Experienced Nurse Practitioners will earn $80-$90 per completed evaluation and receive a mileage stipend along with payment for administrative time.
