Job Description
Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers exceptional flexibility in scheduling and the chance to provide meaningful healthcare to Medicare Advantage members in Bernalillo County, including locations such as Albuquerque, Los Ranchos de Albuquerque, and Tijeras.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule to best suit your availability
- Minimum hourly commitment required on a weekly or bi-weekly basis
Responsibilities:
- Conduct comprehensive annual health assessments in patients homes, each session averaging approximately 45 minutes
- Evaluate patient medical histories, current conditions, and medication regimens
- Educate patients about their health conditions, prescribed medications, and preventive measures
- Identify care gaps or health risks, and deliver recommendations for additional support or follow-up
- Focus strictly on assessment and education; you will not be responsible for developing treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in New Mexico
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months recent Nurse Practitioner experience preferred
