Healthcare professionals passionate about patient-centered care are sought to provide home health risk assessments in Steuben County. This per diem opportunity is ideal for skilled Nurse Practitioners who value autonomy and flexible scheduling.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Qualifications:
- Active Nurse Practitioner license in New York
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited program
- At least six months of current Nurse Practitioner experience preferred
Responsibilities:
- Visit Medicare Advantage plan members in their residences, with assignments in locations such as Corning, Gang Mills, Hornell, Bath, and surrounding Steuben County areas
- Conduct thorough annual home health risk assessments, each averaging about 45 minutes
- Review and evaluate each patients prior and current medical history, conditions, and medication regimens
- Educate members on their medical conditions and prescribed medications
- Identify and document any gaps in care, recommending appropriate follow-up actions to support patient wellness
- No requirement to initiate treatment plans or prescribe medications during assessments
