Job Description
Enjoy the flexibility of a per diem Nurse Practitioner opportunity focused on home health risk assessments for Medicare Advantage plan members. This role involves traveling to patient homes in Chemung County communitiesincluding Elmira, Horseheads, Elmira Heights, and Wellsburgto conduct focused visits that last approximately 45 minutes each. You'll have the freedom to create your own schedule with shifts available any day of the week.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-determined scheduling
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual and post-discharge risk assessments for Medicare Advantage members in their homes
- Review patients' prior and current medical histories, conditions, and medications
- Educate patients about their medical conditions, treatment options, and medication regimens
- Identify gaps in care and make recommendations for improved health outcomes
- No prescribing or treatment plan creation required
Qualifications:
- Active Nurse Practitioner License in New York
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Graduation from an accredited MSN program
- At least 6 months of current Nurse Practitioner experience preferred
This position offers compensation of $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
