Job Description

Experienced Nurse Practitioners are needed to perform home health risk assessments on Medicare Advantage plan members throughout Chemung County, NY. This per diem opportunity is ideal for clinicians seeking flexibility and autonomy in their scheduling while making an impact in community-based healthcare.

Schedule:

  • Flexible shifts are available seven days a week
  • Set your own schedule based on availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual health risk assessments during home visits for Medicare Advantage members
  • Obtain patient histories, review current and prior medical conditions, and evaluate medication usage
  • Educate patients about their medical conditions and prescribed medications
  • Identify care gaps and provide recommendations to help close them
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in the state of NY
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of current Nurse Practitioner experience preferred

Enjoy competitive per-evaluation compensation of $80-$90, plus a mileage stipend and additional pay for administrative time. Locations may include Elmira, Elmira Heights, Horseheads, and Wellsburg, with each assessment typically taking about 45 minutes. The variety of visits includes annual health checks and post-discharge assessments.
