Seeking an experienced Nurse Practitioner for per diem home health risk assessment visits throughout Shelby County, TN, including Memphis, Bartlett, and Collierville. This position offers professionals the autonomy to create their own schedules, with flexible shifts available any day of the week and a minimum hourly commitment required weekly or bi-weekly.

Key Responsibilities:

• Perform comprehensive, in-home risk assessments (approximately 45 minutes per visit) for Medicare Advantage health plan members.
• Evaluate medical history, current conditions, and medication regimens.
• Deliver education to members regarding their health status and medications.
• Spot care gaps and provide recommendations to enhance care, but notethere is no requirement for developing treatment plans or prescribing medications.
• Conduct either annual health assessments or post-discharge visits based on the nature of each assignment.

Qualifications:

• Active Nurse Practitioner license in the state of Tennessee.
• Board Certification through AANP or ANCC.
• Current, valid CPR certification (BLS).
• Master of Science in Nursing (MSN) from an accredited program.
• At least 6 months of recent Nurse Practitioner experience, preferably in home health or a related field.

This role is compensated at $80$90 per evaluation, with additional mileage and administrative time stipends provided.
