Job Description
Make a meaningful impact by delivering high-quality care and preventive services as a Nurse Practitioner providing home health risk assessments throughout Shelby County. This per diem role is ideal for advanced practice clinicians who value flexible scheduling and autonomy while working with Medicare Advantage members in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule based on availability
- Commit to a minimum number of hours per week or bi-weekly
Key Responsibilities:
- Perform comprehensive annual home health risk assessments with an average duration of 45 minutes per evaluation
- Review and document prior and current medical histories, as well as medication usage
- Educate members about their health conditions and medications to encourage self-management
- Identify care gaps and recommend appropriate follow-up, resources, or interventions
- No prescribing or creation of treatment plans requiredfocus on assessment and education
Qualifications:
- Active Nurse Practitioner licensure in Tennessee
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- Current Basic Life Support (BLS/CPR) certification
- Minimum of 6 months experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited school
