Make a meaningful impact as a skilled Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members throughout Allegheny County, PA. This per diem opportunity offers complete flexibility, allowing you to select your own shifts on any day of the week and establish minimum hourly commitments that fit your lifestyle.

Schedule:

• Flexible shifts available MondaySunday
• Choose your own schedule
• Set minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual risk assessments during in-home visits (approximately 45 minutes each)
• Review medical history, current health conditions, and medication usage
• Educate patients about their diagnoses and medications during each visit
• Recognize and address gaps in care, and recommend solutions to promote patient well-being
• Visits may vary: annual health assessments and post-discharge evaluations
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Pennsylvania
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• Master of Science in Nursing (MSN) from an accredited program
• Minimum six months recent NP experience is preferred

Enjoy compensation of $80$90 per evaluation, plus a mileage stipend and payment for administrative time. Youll be scheduled to meet patients in Pittsburgh and across the surrounding areas including Penn Hills and Mount Lebanon, based on shift availability.
