Deliver essential healthcare services in the community by performing targeted home health risk assessments for Medicare Advantage plan members. This per diem opportunity provides the flexibility to create your own schedule and contribute meaningful care through in-home evaluations across White County, including Sparta and Doyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to align with your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes
• Evaluate prior and current medical history, ongoing medical conditions, and medication use
• Educate patients about their health conditions and medications to empower informed self-care
• Identify and document any gaps in care, offering recommendations for follow-up or further evaluation
• No responsibilities related to prescribing medications or creating ongoing treatment plans

Qualifications:

• Active Nurse Practitioner license for the state of Tennessee
• Current CPR/BLS certification
• Board Certification via AANP or ANCC
• Minimum of six months recent NP experience preferred
• Master of Science in Nursing (MSN) degree from an accredited institution

Compensation:

• Earn $80$90 per completed evaluation, with additional provisions for mileage and administrative duties
