Advance your Nurse Practitioner expertise and make a meaningful impact by conducting home health risk assessments for Medicare Advantage members in White County, TN. This per diem opportunity offers maximum flexibilitydesign your own work schedule and focus on thorough patient care in home settings across areas like Sparta and Doyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your preferred days and hours
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform comprehensive home health risk assessments with an average duration of 45 minutes per visit
- Evaluate members medical histories, current conditions, and medication usage
- Educate patients on their health conditions and medications
- Identify care gaps and recommend potential resolutionsno ongoing treatment planning or prescribing required
- Complete relevant documentation during or after visits
Key Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR/BLS certification
- Minimum of 6 months current NP experience preferred
