Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Commit to a minimum weekly or bi-weekly number of hours that fits your needs
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patients homes
- Review and evaluate patients medical history, current health conditions, and medication regimens
- Deliver education tailored to each patients health status and prescribed medications
- Detect potential gaps in care and suggest actionable steps to address them
- No need for developing treatment plans or prescribing medicationsfocus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board Certification with either AANP or ANCC
- At least six months of recent NP experience (preferred)
- MSN degree earned from an accredited nursing program
