Job Description

A vital opportunity is available for a skilled nurse practitioner specializing in home health to support Medicare Advantage plan members. In this per diem role, you will conduct comprehensive home health risk assessments within Cookeville, Algood, Monterey, and neighboring areas of Putnam County. Each visit will average about 45 minutes and may focus on an annual health assessment or a post-discharge evaluation. The nature of this position allows you to make a tangible impactidentifying gaps in care, educating members, and ensuring a higher level of community-based healthcare.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Freedom to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment according to your availability

Key Responsibilities:

  • Complete annual and post-discharge risk assessments in members homes
  • Review and evaluate prior and current medical history, conditions, and medications
  • Offer education regarding medical conditions and prescribed therapies
  • Identify care gaps, recommending solutions and resources as appropriate
  • No prescribing or treatment plan development required

Qualifications:

  • Unencumbered Nurse Practitioner license in Tennessee
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited institution
  • Current BLS (CPR) certification
  • At least 6 months of recent nurse practitioner experience preferred

This position provides compensation of $80$90 per evaluation, plus a mileage stipend and payment for administrative time, reflecting the meaningful contribution you bring to patients lives.
