Schedule
- Flexible shifts available Monday through Sunday
- Customize your own working hours
- Commitment can be fulfilled weekly or bi-weekly
Key Responsibilities
- Conduct thorough, in-home risk assessmentseach lasting approximately 45 minutes
- Review and document patient medical history, conditions, and medications
- Educate patients on managing their health and understanding their medications
- Identify opportunities for improved care and recommend steps for resolution
- No prescribing or treatment planning required
Qualifications
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board certification with AANP or ANCC
- Minimum of 6 months of recent NP experience preferred
- MSN from an accredited school of nursing
