Job Description

Elevate your career as a Nurse Practitioner with per diem opportunities focused on impactful home health risk assessments. In this flexible, patient-centered role, you will conduct in-home evaluations for Medicare Advantage plan members residing across Overton County, including Livingston, Hilham, and Alpine. Each assessment lasts approximately 45 minutes and may cover annual health evaluations or post-discharge visits.

Schedule:

  • Flexible shifts, Monday through Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitments

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage health plan members
  • Review and evaluate previous and current medical history, including medication usage and medical conditions
  • Educate patients on their diagnosed conditions and prescribed medications, promoting understanding and compliance
  • Identify and address gaps in care, offering recommendations to improve health outcomes
  • Note: No prescribing or creation of treatment plans is required

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS)
  • Board certification from AANP or ANCC
  • Minimum of 6 months experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited institution

This per diem position offers competitive compensation at $80$90 per evaluation, plus a mileage stipend and administrative time coverage.
