Deliver essential care in a dynamic per diem opportunity designed for Nurse Practitioners with a passion for home health. Provide comprehensive health risk assessments for Medicare Advantage plan members, supporting individuals in Connersville and throughout Fayette County. This role offers significant flexibilityset your own schedule and determine your preferred level of commitment.
Schedule
- Flexible shifts available Monday through Sunday
- Control your own work schedule
- Commit to minimum weekly or bi-weekly hours as desired
Core Responsibilities
- Perform thorough annual and post-discharge home health risk assessments, each typically requiring about 45 minutes
- Review and evaluate members current and past medical history, medication regimens, and medical conditions
- Educate patients on their conditions and medications to encourage better health outcomes
- Identify gaps in care and provide actionable recommendations toward their closure
- Focus exclusively on assessments with no need to create treatment plans or prescribe medications
Your Qualifications
- Active Nurse Practitioner license in Indiana
- MSN degree from a fully accredited nursing program
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- At least six months of recent nurse practitioner experience preferred
Compensation Enjoy competitive pay of $80-$90 per completed evaluation, with a mileage stipend and reimbursement for administrative time.
