Delivering high-quality care in the comfort of a patients home is deeply rewardingand in this per diem role as a Home Health Nurse Practitioner, youll enjoy the flexibility to balance your professional and personal life. This position focuses on conducting risk assessments for Medicare Advantage members throughout Merrimack County, including Concord, Hooksett, and Bow. Each visit, typically about 45 minutes in length, involves either an annual checkup or post-discharge evaluation, allowing for meaningful interactions and positive impact on individuals health journeys.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitment as fits your availability

Key Responsibilities:

• Perform annual or post-discharge home health risk assessments for Medicare Advantage plan members
• Assess prior/current medical history, chronic conditions, and medication usage
• Educate members about their medical conditions and pharmaceuticals
• Identify care gaps and make recommendations to improve health outcomes
• No treatment planning or prescribing responsibilities

Qualifications:

• Active Nurse Practitioner license in New Hampshire
• MSN degree from an accredited nursing school
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• At least 6 months of recent NP experience preferred

This opportunity allows you to build relationships with patients while working autonomously. Compensation is competitive, with $80$90 per evaluation, along with a mileage stipend and pay for administrative time.
