Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role centers around conducting comprehensive in-home evaluations for Medicare Advantage health plan members throughout Hillsborough County, NHincluding areas like Manchester, Nashua, and Bedford. Each assessment will typically last about 45 minutes, and may involve annual health assessments or follow-up visits after hospital discharges.

Schedule

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Choose minimum weekly or bi-weekly commitments

Key Responsibilities:

• Perform annual home health risk assessments in members residences
• Assess prior and current medical history, overall health status, and medication usage
• Deliver clear, relevant education to members about their health conditions and medications
• Detect gaps in care and offer actionable recommendations to improve patient outcomes
• No responsibility for prescribing medications or generating treatment plans

Qualifications:

• Active Nurse Practitioner license in New Hampshire
• Current BLS (CPR) certification
• Board certification through AANP or ANCC required
• At least 6 months of Nurse Practitioner experience preferred
• MSN from an accredited school of nursing
