Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments, typically 45 minutes per visit
  • Review medical history, ongoing conditions, and medication use
  • Educate patients on their health conditions and therapies
  • Identify care gaps and make recommendations to ensure optimal patient outcomes
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License valid in North Carolina
  • Current Basic Life Support (BLS) CPR certification
  • Board Certification through AANP or ANCC
  • At least 6 months of experience as a Nurse Practitioner (preferred)
  • Master of Science in Nursing (MSN) from an accredited institution
