This per diem opportunity is ideal for a Nurse Practitioner with a passion for preventive care and personalized patient interaction. Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments, typically 45 minutes per visit
- Review medical history, ongoing conditions, and medication use
- Educate patients on their health conditions and therapies
- Identify care gaps and make recommendations to ensure optimal patient outcomes
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License valid in North Carolina
- Current Basic Life Support (BLS) CPR certification
- Board Certification through AANP or ANCC
- At least 6 months of experience as a Nurse Practitioner (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
